GOLD AND PLATINUM PHYSICIAN PRACTICE MEMBERSHIPS:
For practices with fewer than 10 physicians: $5,000 per year
- Up to 10 practice members listed

- Invitation to annual conference
- Recognition in newsletter, conference program book, on the website and in the membership directory

For practices with 11-20 physicians: $10,000
per year

PLATINUM PHYSICIAN PRACTICE MEMBERSHIPS:
For practices with more than 20 physicians: $15,000 per year
